The best online TRT clinics in 2026 are Hone Health (8.2/10), Fountain TRT (8.1/10) and Henry Meds (7.8/10), based on our scoring of price transparency, medical oversight, treatment options and coverage. Every clinic below requires lab work and a prescription, and none bills health insurance.
We checked each clinic's published prices, lab policies, treatment menus and state coverage on September 25, 2026. We haven't bought treatment from any of them; this is research-based. How we score.

1. Hone Health: best overall (8.2/10)
Price: $135/month membership + medication (injections from $28/month)
Hone's membership covers physician consults through Broad Health P.A., lab testing and support; its itemized fee breakdown allocates $78/year to labs (Hone). It has the widest HRT menu we found: injections, cream, troches, enclomiphene, clomiphene and anastrozole. It accepts HSA/FSA cards.
Watch out for: medication is billed on top, so your real total starts around $163/month for injections. Some Trustpilot reviewers say this wasn't clear upfront.

2. Fountain TRT: best all-inclusive price (8.1/10)
Price: $199 per 4 weeks; $167 or $150 per 4 weeks with prepay
One fee covers visits, labs (14+ biomarkers), medication and shipping, and it's the same whether you're prescribed injections, cream or enclomiphene (Fountain). The medical team is led by a board-certified urologist.
Watch out for: 4-week billing means 13 charges a year, and its FAQ lists about 25 states.

3. Henry Meds: best value (7.8/10)
Price: $129/month all-in; $179/month for Kyzatrex oral
Henry includes visits, Quest labs, medication, supplies and shipping in its price, plus anastrozole when prescribed (Henry). It's one of the few online clinics offering Kyzatrex, an FDA-approved oral testosterone (not available in California).
Watch out for: Henry doesn't publish its states or prescribers' credentials. Multi-month plans may require paying the remaining balance if you cancel early.

4. Defy Medical: best for complex cases and peptides (7.8/10)
Price: estimated under $175/month in year one, about $125/month after
Defy is a Tampa-based concierge clinic serving most states by telemedicine, with pay-per-service billing and no subscription (Defy). Its menu includes TRT, peptide therapy, ED treatment and weight loss. It has a 4.8/5 Trustpilot score from 4,000+ reviews.
Watch out for: estimates rather than a price list, so ask for an itemized quote.

Maximus offers four protocols with board-certified physicians, at-home labs and 24/7 care-team messaging (Maximus). It provides receipts for FSA/HSA reimbursement.
Watch out for: the best prices are tied to longer plans, the pricing page doesn't specify included labs, and states aren't published.

6. TRT Nation: lowest sticker price (6.7/10)
Price: $99.99/month + $129 labs
TRT Nation covers injectable testosterone, supplies, shipping and unlimited consults, and serves every state except AL, AK, AR, MO and HI (TRT Nation FAQ). Outside labs are accepted.
Watch out for: a narrow menu (injections and enclomiphene) and a 2.1/5 Trustpilot score from 30 reviews, with complaints about upfront charges.

7. Hims: enclomiphene only, for now (6.6/10)
Price: $99/month (10-month prepaid) to $199/month (3-month plan)
Hims currently prescribes enclomiphene, optionally with tadalafil. Its FDA-approved oral testosterone (Kyzatrex) and injections were still listed as "coming in 2026" on its testosterone page when we checked.
Watch out for: it isn't TRT yet, the lowest price needs 10 months upfront, and there's no HSA/FSA.

Clinics we considered but didn't rank
- Gameday Men's Health: primarily an in-person franchise. Its telehealth FAQ says telemedicine is currently available only in Florida (Gameday).
- Rise Men's Health (eTRT): one of the few telehealth TRT programs that bills insurance, but currently Texas only (Rise). See online TRT that takes insurance.
- Ways2Well: a membership-based precision-medicine clinic ($125/month Core plan) whose site says prescriptions aren't guaranteed or included, and that nine states have limits on telehealth prescribing of controlled medications including testosterone (Ways2Well plans, Ways2Well states). We couldn't verify its TRT medication pricing.

What every legitimate online TRT clinic should do
Use this checklist no matter which clinic you choose:
- Confirm low testosterone with labs. The Endocrine Society's July 2026 statement calls for symptoms plus consistently low testosterone on at least two early-morning, fasting tests, and advises against screening men without symptoms (Endocrine Society).
- Screen for reasons not to treat. The Endocrine Society guideline recommends against TRT in men planning fertility soon, or with breast or prostate cancer, elevated hematocrit, untreated severe sleep apnea, uncontrolled heart failure, or a heart attack or stroke in the past 6 months (guideline).
- Monitor. Expect follow-up labs including hematocrit and PSA.
- Use a licensed prescriber and pharmacy. Testosterone is a controlled substance. The DEA allows it to be prescribed after a video visit under telemedicine flexibilities extended through December 31, 2026 (DEA).
- Know if your medication is compounded. Compounded drugs are not FDA-approved (FDA).

Risks to know
TRT is not a treatment for normal aging. The FDA approves testosterone only for clinically diagnosed low testosterone and, since 2025, requires a class-wide warning about increased blood pressure (FDA). The TRAVERSE trials found no increase in major cardiovascular events but did find higher rates of pulmonary embolism and fractures in treated men (Endocrine Society).
